The PKV open enrollment for trainee civil servants in Germany is a voluntary offer from private health insurers. It is meant to make private cover easier for civil servants with a pre-existing condition or a disability. This guide explains who can benefit, which deadline applies and where the limits of the offer lie.
Short answer: The open enrollment accepts trainee civil servants, explicitly including Referendare (trainees in the preparatory service), with pre-existing conditions into standard tariffs that meet the Beihilfe rules. The formal application must be filed within six months. Risk surcharges are capped at 30 percent of the tariff premium. Last updated: October 2026, the terms of the participating insurer apply.
The most important points in brief
- The open enrollment is a voluntary offer by participating private insurers. It is not based on a law.
- According to the leaflet of the German PKV association, it covers civil servants on probation, for a fixed term and for life. Since 2019 it also covers civil servants on probation for trainees (Beamte auf Widerruf), and trainees and Referendare are named explicitly.
- The formal application must reach the insurer within six months.
- For pre-existing conditions, exclusions are waived. Surcharges are still possible, but capped at 30 percent of the tariff premium.
- If you are entitled to Beihilfe (state health aid), you also need private cover for the remaining costs. The offer relates to tariffs that meet the Beihilfe rules.
- Only the insurer that receives the formal application first must accept you on the easier terms. Get non-binding quotes first, but file the formal application only once.
What is the open enrollment exactly?
Private health insurers check the health status of every applicant. If you have a chronic illness, were treated in the past or live with a disability, you often face risk surcharges or exclusions. The open enrollment addresses exactly this. Participating insurers accept trainee civil servants into standard tariffs that meet the Beihilfe rules, even where they would otherwise charge high surcharges.
The framework matters. The offer comes from individual insurers and is not a legal entitlement. The list of participating companies is published at beamte-in-der-pkv.de. Terms can change, so the current position of the insurer always counts.
For civil servants entitled to Beihilfe, the cover is split in two. The employer pays a share of medical costs as Beihilfe, and private insurance covers the rest. As a standard rate, civil servants on probation for trainees without children receive 50 percent (status October 2026, state Beihilfe regulations). Rates vary by state. The basics are explained in our guide to civil servant health aid.
Who can use the offer, and until when?
The table below summarises the rules from the leaflet of the German PKV association. The leaflet is dated September 2022, and the terms of individual insurers may differ.
| Point | What the leaflet says |
|---|---|
| Eligible group | Civil servants on probation, for a fixed term and for life. Since 2019 also civil servants on probation for trainees, explicitly trainees and Referendare. Closest relatives can benefit too. |
| Deadline | Six months. The period starts with the beginning of the civil service relationship, at the earliest after the end of any preparatory service (Vorbereitungsdienst). |
| Special case | If you were previously a civil servant on probation for trainees with statutory cover, you receive a new deadline when you are appointed on probation. |
| Application | The formal application must reach the participating insurer within the deadline. |
| Start of cover | The leaflet gives no details. |
The wording on the deadline matters for trainees and Referendare. The leaflet leaves open whether the six months only begin after the preparatory service has ended. Clarify this with the insurer before you plan. The guide Health Insurance Between Your State Exam and Training explains how you are insured between exams and preparatory service.
What does the offer give you with a pre-existing condition?
According to the leaflet, participating insurers provide the following in substance:
- a right to be admitted to standard tariffs that meet the Beihilfe rules
- no maximum age for admission
- no benefit exclusions
- risk surcharges of at most 30 percent of the tariff premium
- no health-related surcharge in long-term care insurance (Pflegeversicherung) under the offer
An example shows what the surcharge means. Assumption: the monthly tariff premium is 240 euros and the insurer applies the maximum surcharge. Then 30 percent of 240 euros, that is 72 euros, is the cap, and the monthly premium rises to at most 312 euros. Without a surcharge it stays at 240 euros.
The offer governs admission, but it says little about how well a tariff pays out when you claim. Benefits differ mainly for remedies and medical aids (Heil- und Hilfsmittel). An open catalogue of aids is therefore an important selection criterion. Check carefully what the tariff actually covers.
When does the offer fit, and when does it not?
The offer can make sense if you have a known pre-existing condition and a normal application would bring high surcharges or exclusions. It is not always the best solution, though. Both sides belong in the decision.
The offer may fit if
- you have a condition that would lead to surcharges or exclusions in a normal application,
- you can meet the deadline and file the application on time,
- you find a Beihilfe-compatible tariff with good benefits for remedies and aids.
It fits less well if
- the benefits of the tariff matter more to you than the route into cover,
- you have not calculated the surcharge and the later premium development in advance,
- you want to check whether statutory health insurance suits your situation. It has no health check, but works differently, as the guide Trainee Teachers in Statutory Health Insurance shows.
Whether the offer suits you depends on your condition, your Beihilfe rate and the tariff. A personal consultation with Academycon can go through these points together. More at How we work.
Frequently asked questions
Do I get the offer automatically after my appointment?
No. You must file the formal application within the deadline with a participating insurer. It is not enough that the insurer takes part in the offer. What counts is when the application arrives.
Do I have to disclose my pre-existing condition?
Yes. Information about your health belongs in the application. Incomplete or incorrect statements can cause problems later, even when the policy is already running. Have the questions and your answers explained to you before you sign.
Does the offer also cover my family?
According to the leaflet, closest relatives can benefit as well. Whether and under which conditions this applies to a child or a partner is confirmed by the insurer case by case.
What happens if I miss the deadline?
The special rule then no longer applies, and the insurer’s regular admission terms apply. These often include surcharges or exclusions for pre-existing conditions. It is worth marking the date in your calendar early.
Does the offer apply at every insurer?
No. Only companies that take part in the offer grant the easier terms. Check the current list at beamte-in-der-pkv.de shortly before you apply.
What to do now
- Clarify your start date. Read the beginning of your civil service relationship in your appointment certificate and ask the insurer how it calculates the deadline for trainees.
- Get non-binding quotes from several participating insurers. Compare not only the premium but also the benefits for remedies and aids, for example in the civil servant health aid for trainees context.
- Calculate the surcharge in the worst case. If the maximum premium is affordable for you, you are on the safe side.
- File the formal application on time and keep proof of receipt.
