Doctors in postgraduate training (Ärzte in Weiterbildung) face a combination that many underestimate: health insurance, the professional pension scheme (Versorgungswerk) and disability cover have to fit together. The right answer depends on your salary, on the medical chamber you belong to and on what your Versorgungswerk already provides.

Short answer: Employed chamber members are usually compulsorily members of their Versorgungswerk, the professional pension scheme of the medical chamber. They can under certain conditions apply for an exemption from statutory pension insurance. Health insurance depends on salary: if the annual pay in 2026 is below 77,400 euros, you are compulsorily insured in the statutory health insurance (GKV). The disability pension of the Versorgungswerk is a separate building block.

The most important points at a glance

  • For employed chamber members the Versorgungswerk is a central part of pension and protection.
  • Health insurance depends on pay. The compulsory insurance threshold in 2026 is 77,400 euros a year.
  • An exemption from statutory pension insurance in favour of the Versorgungswerk is possible, but it must be applied for.
  • The disability pension of the Versorgungswerk can already offer protection, but it does not necessarily replace a private disability policy.
  • Changes of post, chamber or federal state alter the situation. Check each post again.

How does the Versorgungswerk work for employed doctors?

The Versorgungswerk is the professional pension scheme of the medical chambers. Using the Berlin Medical Chamber as an example, its information event of 2026 shows: members who are employed are initially compulsorily insured in statutory pension insurance and in the Versorgungswerk, unless they are below the entry age. An exemption from statutory pension insurance in favour of the Versorgungswerk is possible under section 6(1) no. 1 of the Social Code Book VI (in German) and the chamber’s rules. The Versorgungswerk offers, among other things, old-age pensions, survivors’ pensions and a disability pension. The Berlin Medical Chamber describes the benefits in its presentation (status 6 July 2026, in German).

Whether you are a chamber member as a doctor in postgraduate training, and which duties apply, is regulated by the chamber of your federal state. The presentation mentioned does not single out this group, so ask your chamber.

Which health insurance suits postgraduate training?

Health insurance follows your salary. If your regular annual pay is below the compulsory insurance threshold of 77,400 euros (2026), you are compulsorily insured in the statutory system. Only above it can you choose between statutory and private insurance. Pay scales in postgraduate training vary by post, employer and region, so check each post individually. The figures come from the GKV-Spitzenverband fact sheet on the 2026 calculation values (status 1 January 2026).

How does a private disability policy complement the Versorgungswerk?

According to the presentation of the Berlin Medical Chamber, the disability pension of the Versorgungswerk starts with the first contribution payment, and the document states there is no waiting period. Whether and how much a benefit is granted in an emergency depends on the chamber’s statutes and the conditions. A private disability policy can close this gap, but it is a separate decision with its own health check.

With a private policy, look at the health questions, the occupation definition, the referral rules and whether the contract still fits if you move into another specialty. Your career can still change during postgraduate training. A more detailed classification is in our guide on disability insurance during medical school (in German).

When does private health insurance make sense for doctors?

Private health insurance can make sense if your salary is above the threshold and you compare benefits. Many doctors see advantages in the scope of benefits and in the choice of doctors. At the same time, contributions usually rise with age, and the decision is tied to a tariff that you cannot change at will.

If you are employed and privately insured, your employer may pay a contribution. It is governed by section 257 Social Code Book V (in German) and is limited, so ask before you decide. More on PKV during studies and the start of your career is in our guide on PKV for medical students (in German).

Common mistakes when combining these building blocks

The most frequent mistake is to look at the building blocks separately. Anyone who only looks at the health insurance contribution overlooks that the Versorgungswerk already covers part of the protection. A second mistake: the exemption from pension insurance is not applied for, or applied for too late, which can lead to double contributions. A third mistake concerns timing. If you wait too long for disability cover, health questions can make protection harder. Moving between federal states also deserves a look at the statutes of the new chamber before you decide. Ask for an overall summary once a year that brings together the Versorgungswerk, health insurance, disability cover and salary.

Frequently asked questions

Must I join the Versorgungswerk if I am employed as a doctor in postgraduate training?

If you are a chamber member and employed, compulsory membership in the Versorgungswerk generally applies. The exact rule depends on your chamber and your post. The chamber informs you of your duties in writing.

Can I be exempted from statutory pension insurance?

An exemption in favour of the Versorgungswerk is possible. It must be applied for, and the chamber checks the conditions. The decision affects your pension entitlements, so weigh it carefully.

Do I still need private disability cover despite the Versorgungswerk?

This cannot be answered in general terms. The Versorgungswerk offers a disability pension, but the amount and the conditions depend on its statutes. A private policy can close the gap if you check it carefully.

What happens when I change post or move?

A change of federal state or chamber can change responsibility. The presentation of the Berlin Medical Chamber names, among other things, appointment as a civil servant or moving out of the chamber area as ends of membership.

How does health insurance affect my old-age provision?

Health insurance and the Versorgungswerk are separate systems. Compulsion for health insurance follows your salary, while the pension follows the rules of the Versorgungswerk. Plan both together.

What to do now

  1. Ask your chamber whether you are a member and which duties apply to your post.
  2. Check whether an exemption from statutory pension insurance makes sense for you, and apply in time.
  3. Calculate your annual pay and compare it with 77,400 euros to set your health insurance.
  4. Compare the disability pension of the Versorgungswerk with a private policy before you decide.
  5. Have disability, Versorgungswerk and health insurance checked again with every change of post.
