Occupational disability insurance (Berufsunfähigkeitsversicherung, BU) for physiotherapy students has a catch that is often overlooked: the occupation itself works with the body. Those who treat patients lift, hold and stretch all day long. Insurers therefore look closely at the physical demands the job involves. This guide shows how physiotherapy and other health professions are classified.
Short answer: Last updated: October 2026. There is no fixed premium for physiotherapy students. What matters is the physical share of your work and whether you work as an employee or self-employed in a practice. The Federal Statistical Office (Statistisches Bundesamt, Destatis) group human medicine and health sciences recorded about 215,400 students in the winter semester 2025/26 (as of 26.08.2026).
The most important points at a glance
- Under § 172 para. 2 of the Insurance Contract Act (VVG, Versicherungsvertragsgesetz), you are occupationally disabled if you can no longer perform the occupation you last practised. For physiotherapists, the physical share of the work is the core of the assessment.
- Employment in a clinic or rehabilitation centre and self-employment in a practice are classified differently.
- Back, shoulders, knees and hands are typical health topics, both as job requirements and in the application.
- Depending on the tariff, a referral to office work can play a role for physiotherapists.
- The premium depends on your age at entry, your health, your occupational group and the scope of benefits. Only identical benefits are comparable.
How is physical work in physiotherapy classified?
Insurers distinguish between activities with a high physical load and those that mainly involve guidance or organisation. In physiotherapy, the hands-on share is usually large. The table shows typical constellations and the question behind each one.
| Activity | Typical question from the insurer | What to check in the application |
|---|---|---|
| Manual therapy in a practice, as an employee | What share of the working day is hands-on treatment? | State treatment hours and an approximate number of patients |
| Own practice with ownership responsibility | Does treatment or administration predominate? | State shares of treatment, organisation and staff management |
| Clinic or rehabilitation facility | Are there shift patterns and lifting or carrying work? | Describe the duty roster and the tasks concretely |
| Sports physiotherapy | Are there assignments on the pitch or travel? | State locations, times and scope |
| Teaching or further training | Is the activity mainly sedentary? | Note the shares of classroom teaching, preparation and practice |
The table describes typical constellations, not binding classifications. Which occupational group your insurer assigns is set out in the tariff. If you later move from treatment to teaching, check whether the referral clause under § 172 para. 3 VVG covers this change.
Which health questions matter most for physiotherapists?
The application asks about pre-existing conditions, treatments and complaints. People who work in this profession can have back problems, shoulder complaints or joint damage themselves. Such complaints must be disclosed, even if they are well treated. Under § 19 para. 1 VVG, you have a duty to disclose (Anzeigepflicht) the circumstances the insurer has asked about in text form. Gesetze im Internet: § 19 VVG (Last updated: 26.10.2026).
- Note complaints affecting the discs, shoulders, knees and wrists, with diagnosis, treatment and period.
- Also state recurring complaints that appear only with certain movements.
- If a finding is missing, write it expressly in the application and ask, rather than guessing.
A note on this: an insurer can ask additional questions or agree exclusions if the information is incomplete. This matters particularly for physiotherapists, because back and joint complaints are frequently asked about in the application.
What should the pension be for a self-employed practice?
Assume you start as an employed physiotherapist and later open your own practice. Assume you need 1,800 euros a month in case of disability if the practice and private life are to continue. The pension must then cover the practice’s ongoing costs, not only your private needs. These fixed costs are the basis for the pension amount, not the profit of a good year.
How large is the health sciences group?
Physiotherapy and other health professions are usually counted in official statistics in the group human medicine and health sciences. The table compares this group with the two other subject groups mentioned in this guide.
| Subject group, winter semester 2025/26 | Students in total | International students | Share of international students |
|---|---|---|---|
| Human medicine and health sciences | 215,424 | 23,964 | 11.1 percent |
| Agricultural, forestry and nutrition sciences, veterinary medicine | 58,280 | 9,992 | 17.1 percent |
| Art, art studies | 99,200 | 22,567 | 22.7 percent |

Whether physiotherapy degree programmes are fully included in this group depends on how each university is assigned. The figure shows the order of magnitude, not the career path.
Is it worth taking out occupational disability insurance during your studies?
A policy taken out during your studies benefits from a low entry age and usually good health. Against that, physiotherapy students often already work physically during their training, for example in internships, and complaints can arise there. If you have complaints, have them medically clarified before you graduate, because this directly affects the health questions. A personal view of your situation is possible through How we work (in German).
Similar questions about physical strain arise in nursing. The guide on occupational disability insurance in nursing science studies covers this area.
Frequently asked questions
Do I have to state a planned practice founding in the application?
If you plan it concretely, you should state it, because the activity then changes. If the founding is still open, describe your current activity and clarify the question in writing beforehand.
What if I can no longer treat patients after an injury?
Then the insurer checks whether you can carry out another activity that corresponds to your training. This is exactly what the referral clause under § 172 para. 3 VVG is for. Ask in advance whether your tariff includes it.
Is the statutory disability pension enough?
Under § 43 SGB VI, it requires mandatory contributions in the last five years and depends on whether you can work less than six hours a day (partial reduction in earning capacity) or less than three hours a day (full reduction). For a practice owner with fixed costs, it is generally not enough. Gesetze im Internet: § 43 SGB VI (Last updated: 26.10.2026).
Does occupational disability insurance also apply to part-time work?
Whether part-time work is covered depends on the tariff, and some contracts require a minimum scope. Check the scope you state in the application and clarify the consequences of a reduction in writing.
Your next steps
- Describe your career path in the application, with details of hands-on work, shift work and any planned practice founding.
- Have the tariff confirm in writing how it classifies physiotherapy and teaching.
- Check the referral clause under § 172 para. 3 VVG against your career path.
- Answer the health questions completely and keep a copy of the application.
