If you study medicine, you often plan early for a long path to licensing as a physician (Approbation) and into the profession. The question of disability insurance (Berufsunfähigkeitsversicherung, BU) therefore often comes up during your studies. This guide explains why there is no fixed semester from which signing pays off, and which factors count instead: health, tariff rules, financing, and the role of the professional pension scheme (Versorgungswerk) and statutory pension insurance.

Short answer: There is no fixed semester from which disability insurance pays off for medical students. What counts is your state of health, your financial situation and how the training phase is classified in the tariff. Signing early often means better terms for the health details, but it also brings the premium and the tariff conditions into play early. Last updated: October 2026.

The most important points

  • There is no statutory semester. Health, finances and the tariff rules are decisive.
  • During your studies you often have no income of your own. That is why tariffs with their own rules for students are especially important.
  • The classification of the training phase in the tariff also decides what counts as an occupation.
  • After licensing and the start of medical work, membership of the professional pension scheme may become mandatory. This affects statutory pension insurance (§ 6 SGB VI).
  • Treatments from your studies belong in the application if you are asked about them.

Is there a semester from which signing pays off?

There is no fixed limit. The start of your studies is often named as a favourable point in time, because your state of health is usually unremarkable at that stage. Whether signing pays off in the third, fifth or eighth semester, however, depends on the individual case and not on a number.

More important than the semester is the question of what will carry you in an emergency. During training you often have no income of your own that would justify a pension. Whether maintenance, savings or a later salary close the gap decides how much cover you need now. Without a needs analysis, the cover often turns out too small or too large.

What role does the tariff classification play?

The tariff sets out how your training phase is assessed. Some providers distinguish between students, doctors in further training and fully qualified doctors. The definition decides whether an occupational disability is recognised as such in the event of a claim at all. Check therefore which occupational title the contract uses and whether it changes with your licensing.

Under § 172 para. 2 VVG, what counts is the occupation you last practised. During your studies, the degree itself can count as an occupation, provided the tariff so provides. After licensing, the medical activity is the decisive criterion. How this applies in detail is set out in the terms, and this is where you should read carefully. When in doubt, ask the insurer in writing how it classifies the training phase, and keep the answer.

What role do the professional pension scheme and pension insurance play?

After licensing and the start of medical work, compulsory membership of the chamber and the professional pension scheme usually begins. Those who meet these requirements can be exempted from statutory pension insurance (§ 6 para. 1 SGB VI). The exemption is applied for through the professional pension scheme and takes effect from the date the conditions are met, if the application is submitted in time (§ 6 paras. 2 and 4 SGB VI).

Many professional pension schemes provide their own benefits in the event of occupational disability. How high these are and under which conditions they are paid is set out in the statutes of the respective scheme. A statement on the amount is not possible without these statutes, which is why we refer you to the relevant scheme here. A private disability insurance policy can supplement the scheme’s benefit where gaps remain, but it does not replace it.

Which health details are especially important for medical students?

Medical students often have contact with clinics, practices and treatments, partly through compulsory placements and partly through their own illness. In the application you must state everything the insurer asks about. Under § 19 para. 1 VVG, this also covers circumstances that you know about and that you were asked about. Anyone who forgets something risks a dispute about the contract in the event of a claim. This also applies to strains that are not physical.

Dealing with stress is a frequent topic in medical studies. If you have been in counselling or therapy, it belongs in the application, provided you are asked about it. A disclosure does not automatically mean a rejection. It does, however, allow a realistic classification, and that is exactly what protects you in an emergency. Concealment is the bigger mistake.

  • Note treatments, diagnoses and medication for your entire study period, with dates
  • Discuss medical findings that could raise questions later with your GP
  • Answer questions on mental health and sleep as completely as those on physical complaints

What about clinical placements and the practical year?

During clinical placements (Famulatur) and the practical year (Praktisches Jahr, PJ), there may be activities with their own remuneration and their own social insurance. How this affects insurance obligations depends on the specific arrangement. An overview of the course of studies and health insurance is available in Medical Studies: Clinical Placements, Practical Year and Health Insurance (in German). For disability insurance, what matters most is how the activity is described in the application.

Frequently asked questions

Is disability insurance for medical students more expensive than for other courses?

That depends on the tariff, the age at the start of the contract and the classification of the training phase. Blanket statements are therefore not reliable. Compare offers only with the same conditions for medical students, and place the tariff terms side by side. A statement on the prices of individual providers is not possible here.

Do I have to take out a new disability insurance after licensing?

Not necessarily. Many contracts can be adjusted when the occupation changes, for example through guaranteed future insurability or a change of tariff. Check the occasions and deadlines in the contract. Changing without checking can open gaps, especially in the definition of the occupation.

Does the professional pension scheme replace disability insurance?

Not fully. The professional pension scheme regulates its own benefits under its statutes. Whether they cover your needs depends on those statutes and on your personal situation. A private disability insurance policy can close this gap. How large the gap is can only be answered with the actual documents.

Is disability insurance worthwhile if I do not become a doctor?

That depends on whether the policy protects an occupation you actually practise, or whether you abandon medical studies or change your career path. If you drop out or switch, a tariff with a broad occupational description is more helpful than a narrowly defined one. The terms should therefore be read carefully before signing.

So what should you do now?

  1. Note health details, treatments and medication from your entire study period.
  2. Clarify which occupational title applies in the tariff and whether it changes with your licensing.
  3. Check the statutes of your future professional pension scheme as soon as it is foreseeable when membership begins.
  4. Compare offers under the same conditions and decide with the gap in an emergency in mind.
