Your career start as a pharmacist begins after the licence (Approbation) with a position in a public pharmacy, a hospital or the pharmaceutical industry. How much you earn depends mainly on whether a collective agreement applies and what your employment contract says. This guide shows which figures are sourced and how statutory health insurance works from your first job.
Short answer: There is no single nationwide starting salary for pharmacists. In the Nordrhein collective agreement, the ADEXA salary agreement lists EUR 4,089 per month for the first and second year of work (full time, document valid from 1 January 2025). If your annual earnings stay below EUR 77,400 (2026 limit), you are compulsorily insured with a statutory health insurer. Last updated: October 2026.
Key points
- Your salary follows your collective agreement or your employment contract. There is no nationwide fixed figure.
- The Nordrhein agreement lists EUR 4,089 for first and second year staff, made up of a base salary of EUR 3,925 and a bonus of EUR 164. It does not state a new amount for 2026 for licensed pharmacists.
- The ADA salary agreement applies in 2026 across Germany except Nordrhein and Saxony. Check whether it covers licensed pharmacists before you negotiate.
- In statutory health insurance (gesetzliche Krankenversicherung, GKV) the general rate is 14.6 percent plus your insurer’s additional rate, which averages 2.9 percent in 2026. Your employer pays half.
- Occupational disability insurance (Berufsunfähigkeitsversicherung, BU) pays based on the job you last did. Applying early gives you better health terms.
What does a pharmacist earn in the first year?
The most reliable figure comes from the Nordrhein salary agreement, which names pharmacists explicitly. For staff in their first and second year, the document lists a total of EUR 4,089 per month. This total is based on a full-time position of 40 hours per week, with the agreement running from 1 January 2025. The document does not give a figure for a 2026 follow-on agreement, so ask your union or employer for the current position.
Outside Nordrhein, the ADA salary agreement applies in 2026 across Germany except Nordrhein and Saxony, according to the ADEXA overview, valid from 1 January to 31 December 2026. Whether licensed pharmacists sit in which pay grade is set out in the agreement itself. Without a collective agreement, your employment contract sets the pay.
| Source | What it shows | Status |
|---|---|---|
| Salary agreement Nordrhein | EUR 4,089 total in years one and two, full time with 40 hours | Term from 1.1.2025 |
| Salary agreement ADA | Germany except Nordrhein and Saxony, pay grades as set out in the agreement | Valid 1.1.2026 to 31.12.2026 |
| Employment contract | Individual pay, which can also be above the collective rate | At signing |
How do I calculate my social insurance contributions?
Your gross salary is not what lands in your account. Income tax and social insurance contributions are deducted. We do not calculate income tax ourselves. For net examples, use the tax and contribution calculator of the German Federal Ministry of Finance (in German). Health insurance contributions you can calculate directly.
Assumption: gross salary EUR 3,500 per month, statutory health insurance, childless from age 23, status 2026.
- Health insurance share: (14.6 plus 2.9) divided by 2 gives 8.75 percent. 3,500 times 8.75 percent is EUR 306.25.
- Care insurance share: 3.6 divided by 2 gives 1.8 percent. 3,500 times 1.8 percent is EUR 63.00.
- Childless surcharge: 0.6 percentage points, paid by you alone. 3,500 times 0.6 percent is EUR 21.00.
- Total: 306.25 plus 63.00 plus 21.00 is EUR 390.25 per month.
Pension and unemployment insurance are not included in this example. The contribution assessment ceiling for 2026 is EUR 5,812.50 per month, which this salary does not reach. Which health insurance fits the career start?
Employees whose annual earnings stay below the compulsory insurance limit must join statutory health insurance (Section 6 of the Social Code Book V (SGB V), gesetze-im-internet.de (in German)). In 2026 that limit is EUR 77,400 per year, or EUR 6,450 per month. At the Nordrhein starting salary of EUR 4,089 per month you stay well below it. If you earn above the limit and take private cover, your employer contributes a subsidy equal to half the standard rate plus half the additional rate, capped at half of your premium (Section 257 SGB V, gesetze-im-internet.de (in German)).
Statutory insurance offers a single contribution with family cover for children and non-working partners. Private insurance suits people whose income stays above the limit and who want an individually chosen tariff. More background is in Health insurance after graduation: from exams to your first job. How do I protect my income in the first job?
Occupational disability insurance pays if you cannot carry on your last job, as it was before your illness, due to sickness, injury or age-related loss of strength, for an expected long period, fully or partly (Section 172 of the Insurance Contract Act (VVG), gesetze-im-internet.de (in German), paragraph 2). The tariff determines how your job is described. Some contracts also ask whether you could do another activity that matches your training (paragraph 3).
For pharmacists the assessment depends on the activity, for example dispensing, laboratory work or industry. Ask for the tariff wording and check any referral clauses. Before signing, you must answer health questions and disclose pre-existing conditions. Incorrect answers can put benefits at risk (Section 19 VVG, gesetze-im-internet.de (in German)). Frequently asked questions
Will I get a bonus in my first year as a pharmacist?
Whether there is a bonus depends on the collective agreement or your employment contract. In the Nordrhein agreement, EUR 164 is included in the EUR 4,089 total as a bonus. Ask specifically about bonuses, leave entitlement and overtime rules before you accept. A verbal promise is not enough.
Do I have to join statutory health insurance?
If you are an employee and stay below the compulsory insurance limit, yes. Family insurance applies only if your own total income stays below EUR 565 per month (2026) (Section 10 SGB V (in German)).
Does the salary agreement apply to me automatically?
Not automatically. A collective agreement applies to you if your employer is bound by it or if your contract refers to it. Ask before signing which agreement applies and which pay grade you fall into.
Is my health insurance enough if I become unable to work?
No. Health insurance pays for treatment, not for income when you cannot work. For that you need disability insurance or other provision. Benefits depend on the tariff, the start date and the health information you gave.
