The rural doctor quota (Landarztquote) is a separate route into medical school. Applicants receive a place if they commit to working as a general practitioner in an underserved region for a set period after training. Because deadlines, selection criteria and contracts differ between German states, applicants need to know the rules of the state where they apply.
Short answer: The rural doctor quota is a state-level selection procedure for medical study places, linked to a later commitment to general practice in underserved areas. Each German state sets its own application window, usually in spring. In Baden-Württemberg the window for the 2026 procedure closed on 31 March 2026. Last updated: November 2026.
The key points at a glance
- The quota is set by the German federal states, not by a single nationwide rule. Portals, deadlines and criteria are decided by each state.
- In Baden-Württemberg the application window for the 2026 procedure closed on 31 March 2026, according to the state press release.
- The obligation to work in general practice is set out in a contract. According to the state press release of 26 February 2026, it is at least ten years in an underserved area in Baden-Württemberg.
- The school leaving grade alone does not decide. Results in the aptitude test for medical courses, medical professions and volunteer services with patient contact can also count.
- There are no nationwide dates for 2027. Each state ministry publishes its own window.
How does the rural doctor quota work?
The quota is part of the admission process for human medicine. Each state decides how many places it allocates through the quota and regulates the selection in its own state law. Applications therefore do not run through the central allocation procedure, but through a state office, often a ministry or a regional government office. It is a separate path alongside the other routes into medical study.
For applicants this means two things. First, the entry requirements of the state must be checked carefully, because residence and prior training can play a role. Second, the acceptance comes with an obligation that reaches far beyond the degree. Anyone who uses the quota should therefore treat it as a long-term contract rather than as an ordinary study place.
Which deadlines apply in 2026 and 2027?
There are no fixed national deadlines. Each state publishes its own dates, and these can change from year to year. Spring is a typical window, but that is an observation, not a rule. The table below shows what the state releases confirm.
| State | Procedure 2026 | Note |
|---|---|---|
| Baden-Württemberg | Application window until 31 March 2026 | State press release; procedure for 2026 completed |
| Other states | Own dates, sometimes different offices | Check the website of the responsible state ministry |
This guide does not list 2027 dates, because they only follow from an official state announcement. Once a state publishes a window, its own information applies. Applicants who want to be early should set a reminder for January and February and check the websites of the state ministries.
Which criteria count in the selection?
Selection follows the rules of each state. A press release from Baden-Württemberg dated 11 March 2022 names concrete criteria. According to it, the school leaving grade average and a long waiting period are not decisive. Instead, the following points are taken into account. The 2026 press release confirms this line: the grade does not decide alone, while the test result, professional or volunteer experience in healthcare and the selection interview count. The detailed list comes from the 2022 release, so ask the state about changes.
| Criterion | According to the state release of 2022 |
|---|---|
| Result in the aptitude test for medical courses | taken into account |
| Completed training or work in a medical profession | taken into account |
| Voluntary service with patient contact | taken into account |
| Volunteer work with patient contact | taken into account |
| Selection interview on suitability and motivation for general practice | taken into account |
For applicants with a weaker school leaving grade this is important information. The quota is not only a question of grades. Anyone with relevant experience in medicine has different arguments than an applicant who can only point to school certificates. How these routes fit with the rest of the procedure is explained in the guide on study routes without top grades (in German).
Which obligations apply after the degree?
The obligation is the core of the rural doctor quota. According to the Baden-Württemberg release of 2026, after the degree and specialist training, a person must work for at least ten years as a general practitioner in an underserved area of the state. A public-law contract secures this. The 2022 release named a contractual penalty of up to 250,000 euros for breaches; the 2026 release does not mention one, so the amount in the current contract applies. The release does not mention repayment of study costs or a broader duty to settle.
Whether other states provide for similar penalties or repayments is not covered by this release. Therefore, read the contract of the state in full before signing anything. What happens in the case of an early exit is explained in our guide on the contractual commitment (in German).
Step by step: how the application works
- Find the state rule: Check which state offers the quota for your intended place of study and whether you meet the requirements.
- Note the window: Take the application window from the website of the responsible ministry or regional government office into your calendar.
- Prepare the test: A result in the aptitude test for medical courses may count. The announcement of the new test from 2027 is covered in the guide on the TMSnat (in German).
- Collect evidence: School certificates, confirmations of medical activities, voluntary service or volunteer work with patient contact.
- Check the contract: Read the term, the obligation and the penalty before you accept, and send open questions to the state office.
Frequently asked questions
Can I apply with a weaker school leaving grade?
Yes. The Baden-Württemberg press release of 2026 explicitly does not treat the grade average as decisive. Whether this applies in the current procedure only the state can confirm. Applicants should therefore read the current call for applications and not rely on the 2026 release alone.
Does the rural doctor quota exist across Germany?
No. Each federal state decides for itself whether it offers a rural doctor quota and how it is designed. Some states also run quotas for other specialties. An overview by state is in our guide on the quota in the German states (in German).
What happens if I do not meet the obligation?
The rules of the contract apply. In Baden-Württemberg a contractual penalty of up to 250,000 euros is provided for. How an exit works in practice depends on the contract and the individual case. Only the responsible state office can give a binding answer.
Can I apply for other study programmes at the same time?
That depends on the rules of the state and the university. There is no general answer. Anyone pursuing several routes at once should read the application conditions of each office before submitting applications. Our guide on multiple applications (in German) explains the typical situations.
Financing and everyday life in medical school
Those who use the rural doctor quota study human medicine under the same conditions as other medical students. Living costs, rent and health insurance apply, and the course is long. An overview of funding options and typical expenses is in our guide on financing medical study (in German). That guide covers funding, side jobs and ongoing costs, not the selection itself.
For planning, it helps to look early at your own situation. If you have a family, a side job or a previous training, plan the application and the study so that both fit together. An acceptance is a good moment to calculate the financing again before the course starts.
