AOK or Barmer for students is a comparison between a regionally organised family of funds and a nationwide Ersatzkasse with one uniform rate. What matters is therefore which regional AOK is responsible for you, and whether bonus and foreign-travel offers fit your student life. This guide organises the documented details and makes no recommendation.

Short answer: Barmer charges an additional contribution (Zusatzbeitrag) of 3.29 percent. The eleven regional AOKs range from 2.47 to 3.50 percent (fund list of the GKV-Spitzenverband, as of 27 October 2026). AOK Rheinland/Hamburg and AOK Bremen/Bremerhaven charge the same rate as Barmer. On a contribution base of 855 euros the range compared with Barmer runs from minus 7.01 to plus 1.80 euros per month.

The key points

  • Barmer is an Ersatzkasse with one nationwide rate, while AOK is divided into eleven regional funds.
  • Depending on the region, AOK is below, equal to or above Barmer’s rate.
  • AOK Nordost’s StarterBonus names premiums of up to 935 euros per year, tied to conditions (page dated 17 March 2025).
  • Barmer names Reiseimpfungen Plus with 100 percent reimbursement. AOK arranges foreign health insurance with partners.
  • How choosing a fund works legally is explained in switching health insurer as a student.

How do four regional AOKs compare with Barmer?

FundAdditional contributionDifference to Barmer per monthDifference per year
AOK Rheinland-Pfalz/Saarland2.47 percentminus 7.01 eurosminus 84.13 euros
AOK Hessen2.98 percentminus 2.65 eurosminus 31.81 euros
AOK Rheinland/Hamburg3.29 percent0.00 euros0.00 euros
AOK Nordost3.50 percentplus 1.80 eurosplus 21.55 euros

Barmer’s 3.29 percent, or 28.13 euros per month, serves as the reference. The four AOKs shown illustrate the range and say nothing about the AOK in your federal state. AOK determines by postcode whether an AOK is open to you. The background to the term is in additional contribution 2026: all health insurers.

How do fund type and responsibility differ?

AOK belongs to the fund type of the general regional health insurers (§ 4 para. 2 SGB V) and appears in each state as an independent fund. Its articles, bonus and offices are therefore regional. Barmer is an Ersatzkasse with uniform articles, and compulsory benefits are identical at both. For students, AOK points out that many universities have offices near the campus and that proof of insurance goes to the university automatically. At Barmer, enquiries run via phone (0202 568 333 1010), live chat after login, mailbox and call-back.

What do the bonus offers look like?

The AOK details apply to AOK Nordost. Other regional AOKs have their own programmes, which you ask about there. The condition that contributions are not paid by third parties matters for you if someone else covers your contributions, for example through an employer. Have the conditions confirmed in writing.

What applies when travelling and studying abroad?

Barmer names Reiseimpfungen Plus with 100 percent cost reimbursement and points to its own information on studying abroad. According to its student page, AOK offers foreign health insurance as an add-on through cooperation partners, without naming prices. How far your cover reaches when you study abroad depends in principle on whether your enrolment in Germany continues. Ask the fund about its rules for your destination before you leave. The rules for EU countries are explained in student health insurance abroad: Erasmus, USA and work and travel.

Frequently asked questions

Is AOK one fund or several?

Legally there are eleven regional funds, each with its own articles and its own additional contribution. The shared brand does not change that.

Can Barmer turn me away because I am a student?

No. A statutory fund may not refuse membership (§ 175 para. 1 SGB V). That applies to Barmer as much as to the responsible regional AOK, and regardless of your status as a student.

Does long-term care insurance cost something different depending on the fund?

No. The long-term care contribution of 30.78 euros (35.91 euros from the month after the 23rd birthday without children) is the same at every fund.

  2. Compare it with Barmer’s 3.29 percent on an annual basis.
